If there's something important we've left out that you need to let us know about, then you can use the area below to give us extra details.


1. I understand that the Church does not allow its facilities to be used in a way that materially conflicts with its faith.
2. To the best of my knowledge, the purpose for which I am requesting use of the Church’s facilities will not contradict the Church’s faith, and I commit to promptly disclose any potential conflict of which I am aware or become aware of to Church staff.
3. I understand that the Church does not allow its facilities to be generally available to the public, and that my use of these facilities is subject to official designee approval, which is conditioned in part on my agreement to the requirements in the “Church Facility Use Policy,” a copy of which I have read and understood.
4. I understand that upon approval of my facilities use request, I may need to provide a security deposit, a certificate of insurance, and any other fees/documents required by the Church, per “Church Facility Use Policy”.
5. I understand that I will be responsible for any damages to the Church facilities resulting from this proposed use of facilities.
6. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself and my family, and those in my care, while using Church facilities and equipment. On my behalf, and on the behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Church, its employees, agents, and representatives, of and from any claims, including all liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto.
7. I understand that the Church believes disputes are to be worked out between parties pursuant to Matthew 18 and 1 Corinthians 6, without recourse to the courts. Accordingly, I agree to attempt resolution of any disputes through Christian mediation.


1. Walk thru area to insure the reserved area is ready to use.
2. After use, return all equipment, chairs, tables, etc. to their original location.
3. Clean and sanitize tables, wipe chairs and other items, if necessary.
4. If needed, mop and/or vacuum the floors.
5. Collect and take out trash (dumpster is near main entrance on John Exum and just outside the gym
on the lower level)
6. Before departure, check all restrooms in the area approved for use. Make sure all toilets are
flushed and clean if necessary. If soap or paper products are out or if there is another issue, please email Debbie (Debbie.hamm@hbcjc.org) so that she can disseminate info to staff.

NOTE: Cleaning tools/supplies are located in the following areas: breezeway booth closet, multi-center foyer closet, and 800-wing closet (downstairs).
